Crossriver Quality Health Partners

Crossriver Quality Health Partners is a group of healthcare providers from our region who have made a collective commitment to performance improvement by focusing on the quality and efficiency of care for the patients they serve. To support those efforts, they are able to contract together to help achieve their common healthcare goals. Crossriver seeks to:

1) Improve the patient experience, not just patient satisfaction scores but access to healthcare through patient portals and access to non-provider members of the healthcare team who can manage many day-to-day matters of chronic disease management and wellness.

2) Improve the health of the network’s patient population using evidence-based, chronic disease management programs, evidence-based referrals, and wellness programs.

3) Reduce the per capita cost of healthcare through wellness programs, evidence-based referral, screenings, and evidence-based chronic disease management protocols.

4) Improve the work life of health care providers, including that of clinicians and staff.
